A pragmatic approach to lateral hip pain

Greater Trochanteric Pain Syndrome (GTPS) is a common condition that affects up to 25% of women over the age of 40. It presents as lateral hip pain and is most likely due to gluteal tendinopathy. GPS is not only associated with impairments around the hip, but also with significant negative psychosocial impacts including reduced fulltime employment and poor mental health. GTPS can be hard to treat. The current evidence supports interventions such as exercise and appropriate education (load management).

The aim of this course is to provide physiotherapists with:

  1. An accurate understanding of the pathologies that can result in lateral hip pain
  2. The necessary tools to make a diagnosis and evaluate the impairments that someone with lateral hip pain has, and
  3. Provide a range of treatment options for someone with lateral hip pain

The course brings together the current knowledge regarding the aetiology, pathology and risk factors associated with lateral hip pain, and the most recent evidence regarding its treatment.

Having established the likely underlying pathology and risk factors, the course then examines:

  • the known impairments, and discusses how these likely affect the individual
  • how to undertake the diagnostic and functional tests that will guide the physiotherapist in their treatment approach

In addition to recognising the ongoing nature of the pain associated with this condition, the course will cover what is known about the psychosocial issues, noting that some people may need specialist psychological interventions in addition to what a physiotherapist can offer.

Various treatment approaches are then discussed, demonstrated and practiced.

Friday night lecture: 6:00 pm - 8:00 pm

Saturday course: 8:30 am - 5:00 pm

Learning outcomes

  • Understand and know the underlying aetiology, pathology, risk profile of someone with GTPS.
  • Recognise the complexities associated with a person who presents with GTPS.
  • Know the key tests to undertake to differentiate between lumbar spine, SIJ and a hip condition.
  • Perform the clinical tests to differentiate lumbar spine, SIJ and hip diagnosis, and understand their diagnostic value.
  • Perform the clinical tests to differentiate between intra and extra articular hip conditions and understand their diagnostic value.
  • Interpret functional assessments with respect to hip OA and GTPS (including gait and single leg stance).
  • Understand and implement the general principles for managing GTPS, including client centred goal setting.
  • Understand and be able to deliver appropriate education to facilitate the treatment and management of GTPS.
  • Be able to provide short term treatment options.
  • Be able to provide long term treatment options.

Prerequisites

  • Physiotherapists with some experience of treating backs and hips, and who are seeking to gain further understanding and skills in assessment and management of conditions around the hip. This course is only available to qualified and AHPRA registered physiotherapists.

Angie is an international expert in the field of hip pain. In addition to her 29 years of clinical experience she has a number of publications in international peer reviewed journals and has presented at international and national conferences. Angie has committed the last 14 years of her life to the study of hip pain related problems so as to improve the lives of those who suffer from it. In the mid 2000s she and Professor Paul Smith recognised that many people were being misdiagnosed in relation to the cause of their hip pain. Angie undertook to positively affect this situation, undertaking a series of research projects that led to a PhD which examined the difference between hip osteoarthritis and bursitis/greater trochanteric pain syndrome. Prior to completing her PhD in 2011 (Medical Science, ANU) Angie completed a Master's degree in 1999 (Physiotherapy, UNISA) and a Bachelor's degree in 1985 (Physiotherapy, Lincoln Institute of Health Science). Angie established Hip Physio in 2013 after returning in late 2012 from being a Post-Doctoral Fellow at the Centre for Hip Health and Mobility, University of British Columbia, Vancouver, Canada. She currently holds an appointment as Professor Paul Smith's Clinical Assistant, is an Adjunct Associate Professor at the University of Canberra (Physiotherapy School) and is a visiting physio at the Australian Institute of Sport. She has previously worked at TM Physio in Deakin, Corinna Physiotherapy Centre in Woden, Margaret O'Donovan's City Physiotherapy, Di Keddie's Physiotherapy practice in Dickson and the Monash Medical Centre in Victoria. Angie is a member of the Australian Physiotherapy Association and contributes to this by being the ACT State Chair and ACT representative on the National Orthopaedic Network committee.
