Low Back Pain in the Athletic & Young

The purpose of this 60 minute presentation is to:

Identify two clear subsets of back pain seen among young athletes
1. Posterior column stress reactions (mostly spondylolysis)
2. A subset of anterior column pathologies: Mechanical discogenic pain

These two subsets are illustrated by two different sporting codes:
1. Fast Bowling in Cricket
2. Rowing


Fast bowling is (among other loads) a repetitive end range extension / rotation load sport, and rowing is a repetitive & sustained flexion load sport.
The diagnosis of posterior column stress reactions and reversible posterior disc protrusions is possible with clinical examination. Stress reactions are provisionally diagnosed clinically by confirmation requires high tech imaging.
Treatments for the two conditions are very different and are based on diagnosis.

The lesson will cover
1. Prevalence of LBP in different sporting codes
2. Spondylolysis versus spondylolisthesis
3. Mechanical discogenic pain common in rowers
4. Case studies.
5. Discussion with emphasis on rejection of the Non-specific label and associated management systems

Dr Mark Laslett, PhD, NZRPS, Dip.MT, Dip.MDT is a New Zealand Board Registered Musculoskeletal Physiotherapy Specialist (NZRPS) registration number 70-02210. He has been in continuous musculoskeletal clinical practice since 1971. He completed doctoral studies at Linköping University, Sweden (2005), based on diagnostic accuracy research with chronic low back patients undergoing interventional spinal diagnostic procedures with the late Dr Charles Aprill MD in New Orleans. He is a former instructor for the New Zealand Manipulative Physiotherapists Association (NZMPA), Auckland University of Technology (AUT), and former international instructor for the McKenzie Institute International. He has been a tutor in biostatistics and study design for AUT University, and supervisor/advisor for masters and doctoral students undertaking diagnostic accuracy research. He retains an adjunct status with AUT. Mark has taught short courses internationally since 1985. Between 1985 and 1992 this was as a senior international instructor for the McKenzie Institute International. He commenced teaching his own upper and lower limb courses internationally in 1993 until 2004. Since then, his teaching has focused on the lumbar spine, pelvis and hip, as well as the general principles of the science of diagnostics. He has taught extensively in Scandinavia, Europe, North America, and Australasia. In 2019 he taught one course in Taiwan, and commenced teaching in South America (Chile, Brazil & Argentina).Mark is an honorary life member of the New Zealand Manipulative Physiotherapists Association (NZMPA), Physiotherapy New Zealand, and the Swedish branch of the McKenzie Institute and past president of the NZMPA.Mark is in part-time specialist musculoskeletal practice and a triage consultant for the Canterbury Initiative Low Back Pain Health Pathway. In 2015 he co-founded “Southern Musculoskeletal Seminars” with Dr Angela Cadogan, which provides structured online education courses for clinicians on musculoskeletal diagnostics and pain science. As of November 2020, Mark ceased having a financial interest in SMS while retaining his status as a content provider.Mark has over 40 scientific publications, many of which relate to diagnosis and management of low back pain. His book “Mechanical Diagnosis & Therapy: The Upper Limb” was published in 1996
