Acute Dizziness in the Emergency Department

Acute dizziness is a common presentation to the emergency department. Given emerging advanced practice physiotherapy roles which operate within this dynamic environment, physiotherapist are more frequently being asked to consult on dizzy patients presenting to the emergency department. This webinar series aims to provide a theoretical introduction to assessing and treating the acutely dizzy patient. It will equip primary contact physiotherapist with the knowledge for differentially diagnosing between acute and episodic vestibular syndromes, identify clinical red flags in neuro-otological testing and promote skills in point of care repositioning techniques.

Learning outcomes

  • Have a basic understanding of the anatomy of peripheral and central vestibular system.
  • Be able to differentiate between acute and episodic vestibular syndrome using the ATTEST triaging framework and EVESTA algorithm.
  • Have a sound understanding of disorders affecting this vestibular system.
  • Be familiar with acute bedside neuro-otological testing of the dizzy patient.
  • Have a sound understanding of clinical red flags in acute dizziness.
  • Have a basic understanding of nystagmus and its relationship to extraocular muscles
  • Be able to confidently identify peripheral vs central nystagmus in oculomotor and positional testing.
  • Have a theoretical knowledge of typical BPPV assessment and treatment including positional testing, nystagmus patterns and appropriate repositioning manoeuvres.

Prue Neely


Prue is a senior physiotherapist with over 15 years' experience in public and private health services. For the past 8 years Prue has had a focused practice on vestibular medicine with a special interest in acute dizziness. She has worked as an advanced practice emergency physiotherapist since 2015 and continues to work alongside emergency physicians to facilitate timely diagnosis and treatment of vertigo and dizziness. Prue graduated from Charles Sturt University in 2009 with a Bachelor of Physiotherapy and obtained a Master in Health Management from University of NSW in 2020. She completed the Basic (2016) and Advanced (2021) Susan Herdman vestibular competency courses. Prue has locum in balance clinics at Guys Hospital, London and was selected as a boxing ringside physiotherapist for the 2016 Commonwealth Games. Prue implemented the first vestibular rehabilitation outpatient service on the Central Coast in 2014 and has recently opened a dedicated private vestibular clinic-The Dizziness Directive.Prue has published multiple peer review papers on the acute management of BPPV within the emergency department with the Emergency Medicine Australasia Journal and developed a diagnostic algorithm for emergency physicians to utilise in acute dizziness presentations. This work has received awards from international publishing houses and from the health district and has been cited in international consensus statements. It is currently used in emergency departments throughout NSW Health to guide differential diagnosis of vestibular presentations. In addition to these publications, she is a reviewer for the BMC Journal and has presented at the International Emergency Medicine Conference, National Allied Health Conference and international keynote speaker at the Myanmar Physiotherapy Association with abstract submissions to Neuro Otology Society of Australia (NOTSA) and Australian College of Emergency Medicine (ACEM) and Barany Society.
