Chronic disease and comorbidities in diverse populations

Over 80% of older Australians have at least one chronic disease. Subsequent increased healthcare utilisation and burden of care, means heavy Physiotherapist involvement in recovery from hospitalisations including assessment, treatment and discharge planning. This presentation will cover concepts of chronic disease, multimorbidity, comorbidity and frailty for diverse populations, including those who identify as ATSI, and evidence for their Physiotherapy care.

Jacqui Kay and Hannah Crowley are the clinical leads for General Medicine and Aged Care at The Royal Melbourne Hospital and have a passion for the care of vulnerable populations. Both have postgraduate qualifications and successful grants in improving the care of Australian's with chronic disease including diabetes and dementia/delirium.
