Functional vs structural pelvic floor disorders: When does surgery help?


Colorectal pelvic floor disorders often sit at the intersection of functional dysfunction and structural abnormality, yet distinguishing between the two is critical when considering surgical intervention.

This session will provide an overview of common conditions, including obstructed defaecation, rectocele, haemorrhoids, fissures, intussusception, rectal prolapse, OASI and faecal incontinence. Through practical clinical examples, we will explore how symptoms, examination findings, and imaging are interpreted in a surgical context, and when structural abnormalities are likely to be clinically relevant.

The focus will be on when surgery may be beneficial, when it is unlikely to improve symptoms, and the common pitfalls in patient selection—particularly in the presence of functional disorders.

Presenter:
Dr Matthew Trinder is a colorectal and general surgeon who has recently returned to Perth following advanced colorectal training in New South Wales through the Colorectal Surgical Society of Australia and New Zealand (CSSANZ). He holds appointments at Sir Charles Gairdner Hospital, Osborne Park Hospital and Hollywood Private Hospital.

His clinical interests include minimally invasive colorectal surgery, pelvic floor dysfunction and defecatory disorders. He has additional training in anorectal physiology and endoanal ultrasound, and has a strong interest in collaborative management of complex pelvic floor disorders.
